Supported Community Living (SCL)

Individuals in our program may live in their own home or apartment or perhaps with the family. Programs are exceptionally tailored to meet the unique needs of the individuals such as finding a place to live and the money to pay for the living expenses. A plan is developed for each individual, outlining their support needs, as well as specific outcomes expressed by the individual, or evaluated as an area of need/challenge. The plan is then used to help each individual progressively strengthen their individual knowledge, skill and independence.

SCL services may require round the clock 24 hour staffing or minimal meetings with staff per week. This all depends on the unique needs of the person. Services are provided in Polk, Dallas, Warren counties.

What We Offer

Our SCL program offers the following services:

  3. Mental Health symptom Monitoring

  4. Stress Management

  5. Socialization skills and providing access to community based activities

  6. Budgeting and training on Money management skills

  7. Representative Payee for Social Security

  8. Meal Planning and meal preparation

  9. Assistance with Assistance with obtaining and maintaining entitlements. (For example: food stamps, utility discounts, fuel assistance, section 8 housing, Medicare, Social Security and other governmental services.)

  10. Providing ADL trainings

  11. House Cleaning-Skill Building with visual aids

  12. Social Integration

  13. Health Lifestyle instruction and classes
